Who Qualifies for Weight Loss Injections in Northern Ireland?

Who Qualifies for Weight Loss Injections in Northern Ireland?
This is probably one of the questions we get asked most.
Someone has heard about Mounjaro or Wegovy, they know a friend who is using it, or they have seen people talking about weight loss injections online and they want to know one thing:
Would I actually qualify?
If you live in Northern Ireland, the answer depends on whether you are looking at private treatment or trying to access help through the HSC.
The criteria are not the same, and that is where a lot of the confusion comes from.
What BMI Do You Need for Private Weight Loss Injections?
For adults, the licensed criteria for both Mounjaro and Wegovy are fairly straightforward.
They are licensed for weight management in people with:
- a BMI of 30 or above, or
- a BMI between 27 and 29.9 if they also have at least one weight-related health condition.
Examples of those conditions can include high blood pressure, high cholesterol or triglycerides, sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.
That does not mean everybody with a BMI over 30 automatically gets treatment.
It means you may meet the basic weight criteria for a clinician to consider it.
There still needs to be a proper medical assessment before anything can be prescribed.
So If My BMI Is Over 30, Am I Guaranteed Treatment?
No.
BMI is only one part of it.
The prescriber also needs to look at your medical history, current medication, previous treatment and anything else that could affect whether a particular medicine is suitable for you.
Two people could have exactly the same BMI and get different clinical decisions.
That is normal.
Prescription weight loss treatment should not work like buying something from a normal online shop. The clinician has to be satisfied that the medicine is appropriate for the person in front of them.
The MHRA has also warned people against buying GLP-1 medicines from unregulated sellers. These medicines should come from a legitimate pharmacy with a prescription issued by a healthcare professional.
What If My BMI Is Between 27 and 30?
You may still meet the licensed criteria, but there normally needs to be a weight-related health condition as well.
For example, somebody with a BMI of 28.5 who also has high blood pressure may potentially meet the licensed criteria, whereas somebody with the same BMI and no relevant health condition may not.
This is why it is worth completing an assessment rather than trying to work it all out yourself from one number.
The clinician looks at the whole picture.
How Do I Work Out My BMI?
BMI is calculated using your height and weight.
You do not need to get too caught up in the maths. Most online BMI calculators will do it for you in a few seconds.
You will normally need:
- your current weight
- your height
Try to use a reasonably recent weight rather than what you weighed six months ago.
BMI is not a perfect measure and it does not tell us everything about somebody's health, but it is still commonly used when assessing eligibility for weight-management treatment.
What About Weight Loss Injections Through the HSC in Northern Ireland?
This is where the criteria are very different.
Northern Ireland is now introducing its first Regional Obesity Management Service, known as ROMS.
The service is being rolled out in phases and is initially aimed at people with the highest clinical need.
Current official Northern Ireland guidance says phase one is for adults aged 18 and over with a BMI above 45 and at least one of the following:
- cardiovascular disease
- high blood pressure requiring medication
- raised cholesterol or triglycerides
- confirmed obstructive sleep apnoea
- type 2 diabetes
Weight loss medication may form part of the care offered where it is clinically appropriate.
So somebody who may qualify for treatment privately will not necessarily meet the current criteria for ROMS.
That is an important distinction.
What Is OPIP?
You may also have heard about OPIP, the Obesity Pathway Innovation Programme.
This is another new weight-management programme in Northern Ireland.
According to the current NI Direct guidance, adults with a BMI between 30 and 44.9 may be eligible for OPIP. It offers structured weight-management support through a mixture of digital services, clinical teams, community pharmacies and local programmes. Some parts of the service depend on where you live.
OPIP is not simply a route to getting weight loss injections.
It is a wider weight-management programme.
That is worth making clear because the different schemes can sound very similar when you first read about them.
Can My GP Prescribe Weight Loss Injections in Northern Ireland?
The position in Northern Ireland has been changing as the new obesity services are introduced.
Historically, Northern Ireland did not have the specialist weight-management service needed to provide medicines such as Wegovy through the HSC. The Department of Health has now started rolling out ROMS specifically to address that gap.
So if you are looking for treatment through the public health service, access depends on the current HSC pathway and whether you meet the criteria being used during the phased rollout.
Private treatment works differently.
You can usually begin with an online clinical assessment without first needing a GP referral.
What If I Have High Blood Pressure or High Cholesterol?
These conditions do not automatically prevent you from receiving treatment.
In fact, they are examples of the weight-related health conditions considered within the licensed criteria for some patients with a BMI below 30.
That does not mean treatment will automatically be approved.
The clinician will still look at how the condition is being managed, what medication you take and anything else relevant to your health.
The important thing is to be honest on the assessment.
Leaving information out because you think it might stop you qualifying is never a good idea.
What If I Have Type 2 Diabetes?
Again, it depends on your circumstances.
Type 2 diabetes is one of the recognised weight-related conditions included within the licensed indications for these treatments.
But diabetes medication can complicate things, so your treatment history and current medicines are particularly important.
Do not assume that because someone else with diabetes is using Mounjaro, you should automatically be using it too.
The clinician needs to assess you individually.
What If I Have Used Weight Loss Injections Before?
Tell the clinic.
This is especially important if you are moving from another provider or coming back to treatment after a break.
The prescriber will normally want to know:
- which treatment you were using
- what dose you reached
- when you last took it
- whether you experienced any problems
Do not simply assume you should restart at the dose you finished on.
The MHRA advises people to speak to a healthcare professional before switching between GLP-1 medicines because the medicines and doses are not automatically interchangeable.
Does My Age Matter?
Yes, because each medicine has its own licensed use and the clinic itself may have its own service criteria.
For Keltoi's online weight-management service, we assess adults aged 18 and over.
Age alone does not decide whether you qualify, but it is one of the things considered as part of the wider assessment.
Are Weight Loss Injections Suitable for Everyone?
No, and they should not be.
There will always be people for whom a particular medicine is not suitable.
Your health history, current medication and individual circumstances all matter.
This is why a proper clinical assessment is needed before prescribing.
If the clinician needs more information, they may ask follow-up questions or occasionally advise that you speak to your GP before going further.
That can be frustrating when you just want to get started, but it is exactly what you want from a responsible prescribing service.
Does Living Outside Belfast Make Any Difference?
No.
If you are using an online private clinic, you do not need to live in Belfast to complete an assessment.
People in Derry, Lisburn, Newry, Bangor, Ballymena, Coleraine, Omagh, Enniskillen and elsewhere in Northern Ireland can complete the same online process.
That is one of the main benefits of remote weight-management care.
You are not relying on having a specialist clinic five minutes from your house.
Do I Need a GP Referral for Private Treatment?
Usually, no.
You can begin with the clinic's own assessment.
The prescriber then decides whether treatment is appropriate based on the information provided and any further checks they need to make.
There may be individual situations where the clinician wants information from your GP or advises you to speak to them, but you do not normally need to wait for a GP referral just to begin a private assessment.
What Happens During the Assessment?
It is not particularly complicated.
You will usually be asked about your height and weight, medical history, current medication, previous weight-management treatment and anything else the prescriber needs to know.
The point is not to catch you out.
It is to make sure treatment is appropriate.
At Keltoi, the assessment is completed online and then reviewed by a UK prescriber.
If treatment is clinically suitable, the next steps can then be arranged.
What If I Do Not Qualify?
It does not mean there is nothing you can do.
You may simply not meet the criteria for prescription treatment at the moment.
Depending on your circumstances, other approaches to weight management may still be appropriate.
And if you are close to one of the BMI thresholds, do not try to gain weight simply to qualify for medication.
That completely defeats the purpose.
If you are unsure what your options are, speak to a healthcare professional.
Is Private Eligibility Different From HSC Eligibility?
Yes, and this is probably the main thing to take away from this article.
For private treatment, medicines such as Mounjaro and Wegovy are licensed for adults with a BMI of at least 30, or at least 27 with an appropriate weight-related health condition.
For the current first phase of Northern Ireland's ROMS service, the threshold is much higher: BMI above 45 plus at least one specified health condition.
There is also OPIP, which currently covers a broader BMI range of 30 to 44.9 for eligible adults, but that is a wider weight-management support programme rather than simply a prescription service.
That is why two people can both be told completely different things depending on whether they are asking about private treatment or the HSC.
The Bottom Line
If you live in Northern Ireland and are wondering whether you qualify for weight loss injections, your BMI is the first place to start, but it is not the whole answer.
For private treatment, you may meet the basic licensed weight criteria if your BMI is:
30 or above, or
27 to 29.9 with a weight-related health condition.
From there, a prescriber still needs to assess whether treatment is safe and appropriate for you.
Public access through Northern Ireland's new HSC weight-management services currently has different and more restricted criteria, with ROMS being introduced first for people with the highest clinical need.
At Keltoi, adults in Northern Ireland can complete an online weight-management assessment which is reviewed by a UK prescriber.
If treatment is suitable, the available options can then be discussed.
And if it is not suitable, the clinician should tell you that too.
That is how it should work.
This article is for general information only and does not replace individual medical advice. Prescription treatment is subject to clinical assessment and suitability.
